Default Page Size
The default page size is A4. Sorry this is a US-centric question... Is it possible to permanently change this to Letter? Is it possible to remove the ISO page size options? -- BrettCannon - 14 Jun 2016 In case you are using NatSkin the default page size can be set using the%PDFPAGESIZE preference setting.

Thanks! After posting my question, I also found that the print dialog template foswiki/templates/print.nat.tmpl can be modified.
