Calendar does not pop up on date field

I'm trying to use a date field with EditTablePlugin, and the calendar does not pop up when I enter the date field. Below is a simplified version of the test table I created (I'm also trying to use the date format string, etc, but I wanted to limit the number of things I could be doing wrong):

edit table date test
Name Birthdate

I tried accessing this through several different computers and browsers, so I don't think it's a client-side issue.

Thanks in adance.

When I click the EditTable button on your example above, the calendar link to set the date appears and is usable.

Could you report back some more information on the versions of your installed plugins for EditTablePlugin, JSCalendarContrib

Update 12/10/2011: I verified that it does work in my browser from this website, but not when I paste the exact same wikitext on my website.

I'm running Foswiki-1.1.3, Sat, 16 Apr 2011, build 11475, Plugin API version 2.1, EditTablePlugin 4.40 (latest) and I just upgraded JSCalendarContrib to 1.5.0, with no changes in the results.

I do see a note on JSCalendarContrib that says "Reverted back to June 2011 releaseThis plugin is compatible with Foswiki 1.1.3 There was a possible date corruption issue with version 1.4.x if a custom date format was chosen that was not understood by Foswiki core date functions. This update reverts those changes."
