While copying headlines from the page to the %TOC, HTML markup may contain ID attributes. These must be unique in all of the page. Thats even more important as <a name="foo"></a> is deprecated in HTML5 in favor of using IDs for anchor targets like in <a id="foo" href="...">headline</a>
