I noticed this a few times, seems to be related with automatic upload by build script to twiki.org. Here is what is wrong:

Arthur created r1 of TWiki:Plugins/TopicDataHelperPlugin
I added a form and fixed some content (in Plugin Info table), creating rev 2: http://twiki.org/cgi-bin/view/Plugins/TopicDataHelperPlugin?rev=2
The diff does not show any change: http://twiki.org/cgi-bin/rdiff/Plugins/TopicDataHelperPlugin?rev1=2;rev2=1
Arthur did some more changes, creating rev 3; the rev 2 is missing from rdiff: http://twiki.org/cgi-bin/rdiff/Plugins/TopicDataHelperPlugin?rev1=3;rev2=1

No reliable way to reproduce. Put it down to the server.
