Sandbox/InProcessForm.txt is missing from the release archive, so the example doesn;t work all the way through the flow

Also there is a lack of defensive programming in the failure to check the return result from _initTOPIC. If the form is missing this could lead to failures.

Added the InProcessForm, and strengthed the defensiveness of the code.

Opened again because it doesn't redirect correctly when a barred user makes a transition.
