Item812: FormPlugin element tab order is random

at minimum make it sequential, with submit and cancel buttons a part of the order

at the moment its random, and submit isn't there (or is so far away i didn't find it)

worse is that the jscalendar steals the sumit, so if you have a text entry and hit enter, you get the jscalendar popup, not submit.

Added tabindex to form elements and submit. No fix for the jscalendar stealing though.
