With validation disabled - which is needed for upload according to the documentation, the URL is cleared. The following fix resolves the issue:

(10:52:20 AM) gac410: Change line   undef $actionUrl if $disableValidation;   to
(10:52:27 AM) gac410: undef $actionUrl if ($disableValidation && ! $action eq 'upload' );
