You cannot change the comment of an existing attachment using the "Change comment and properties only" button.

Reproduce: create new topic-> Attach -> select file -> upload -> click on "manage" from attachment -> change comment -> "Change comment and properties only" Result: no change to the attachment expected: comment is adopted

I was also able to reproduce this in sandbox. https://foswiki.org/Sandbox/TestMg
